The RN Case Manager will play a pivotal role in operationalizing this model — serving as the clinical and administrative liaison between client practices, patients, and the MSO’s central leadership team.
Position Summary
The RN Case Manager is the frontline leader responsible for coordinating care, managing chronic disease workflows, optimizing ancillary testing programs (ANS, BrainView, Echo, Carotid, LEA/LEV, AAA, PFT, HST), and driving operational performance for assigned client practices.
You will work closely with physicians, nurse practitioners, and office teams to implement the SERVE MD Operating System, integrating technology, remote staff, and automation to maximize quality and efficiency.
Core Responsibilities
1. Care Coordination & Patient Management

Manage high-risk and chronic disease patients (e.g., CHF, COPD, diabetes) using data-driven protocols.

Oversee care transitions, medication reconciliation, and follow-up appointments.

Ensure all Medicare wellness visits, labs, and diagnostics are completed on schedule.

Drive patient engagement initiatives via text, email, and remote monitoring programs.
2. Practice Optimization

Audit clinical workflows and identify revenue-leakage or inefficiency points.

Coordinate implementation of in-office diagnostics (ANS, BrainView, vascular and cardiac testing).

Collaborate with billing and operations teams to ensure accurate documentation and coding.

Train front-office and MAs on new SOPs and Serve MD OS protocols.
3. Leadership & MSO Integration

Serve as the MSO’s on-site ambassador for assigned practices (5–15 per RN).

Report performance metrics (EBITDA lift, visit volumes, patient compliance) to MSO leadership weekly.

Partner with virtual assistants, scribes, and AI-enabled automation systems to streamline daily operations.

Support provider burnout reduction by standardizing workload and leveraging remote staff.
4. Quality & Compliance

Maintain adherence to CMS and state regulatory standards.

Monitor quality measures (HEDIS, MIPS, CCM, TCM).

Ensure HIPAA compliance across patient communications and technology use.

Assist in accreditation and audit preparation as required.
Required Qualifications

Registered Nurse (RN) – active, unrestricted license (multi-state compact preferred).

Minimum 3 years of clinical experience (Internal Medicine, Family Medicine, or Geriatrics preferred).

Prior experience in case management, population health, or care coordination required.

Strong understanding of EHRs (eClinicalWorks preferred) and virtual care technology.

Excellent communication, problem-solving, and leadership skills.
Preferred Skills

Experience with Medicare Advantage, CCM/PCM, and value-based care programs.

Familiarity with AI-based workflow automation, telehealth platforms, and CRM systems.

Ability to analyze performance dashboards and recommend process improvements.

Compensation & Benefits

Competitive base salary ($85,000 – $110,000 annually DOE).
