Provide quality nursing care for patients in home setting, Assess patient, families, and caregivers' ability to manage patient care. Teach patient, families, and caregivers' in the care of long/short term illness and wound care, Determine intermittent visits based on acuity and level of care needed., Complete re-certifications, resumption of care, transfers, and discharges., Consult with and educate patient and family regarding the disease process, self-care techniques, and prevention strategies., Coordinate all patient/family services and prioritization of needs with the members of the interdisciplinary team: OT, PT, MSW, and speech therapist., Inform the physician and other personnel of changes in the patient status, needs and outcomes of interventions., Bilingual English/Russian
Job Duties:
Take care of the patients in home settings and Assistant Living Facilities. Provide case management. Conclude comprehensive admission, assessment of patients overall conditions, in order to develop personalized care plans. Evaluate patients status, change in condition, and/or medications and update plans of care on an as-needed basis. Supervise home health aides and Licensed practical nurse to establish optimal care for patients. Provide education for HHA, building POC for HHAs.
Education Requirements:
Must have a Associate’s Degree
Must have a High School Diploma
Good to have a Bachelor’s Degree
Skill Requirements:
Must have 0-1 years of experience in Provide quality nursing care for patients in home setting
