Job Duties:

Position Summary

Tenderness Health Care is a trusted provider of compassionate home health services and has proudly served the community for over a decade. As part of our continued growth, we are seeking a Registered Nurse (RN) to join our team. This role involves traveling to patients’ homes to assess patient conditions, manage care plans, and support caregivers while ensuring high-quality, patient-centered care.

Responsibilities

  • Travel to patient homes to manage and follow care plans according to physician instructions

  • Review patient records prior to visits and conduct in-home nursing assessments for recertification or changes in condition

  • Conduct bi-monthly supervisory visits to evaluate Personal Care Workers (PCWs) and ensure proper care delivery

  • Document patient condition changes and provide training or guidance to caregivers as needed

  • Complete required documentation including PCST, PCA, and 485 forms within 48 hours of recertification or amendment visits

  • Educate patients, caregivers, and family members on ongoing care and aftercare instructions

  • Communicate patient progress and recommendations to physicians and care team members

  • Monitor patient recovery and maintain accurate clinical documentation

  • Respond to phone, email, or text communications within 24 hours or sooner if urgent

  • Attend team meetings and support office staff when needed

  • Follow company policies, procedures, and compliance standards

  • Perform other duties as assigned

Qualifications

  • Active Registered Nurse (RN) license in the applicable state

  • Previous home health or community nursing experience preferred

  • Strong assessment, documentation, and communication skills

  • Ability to travel to patient homes and work independently

Education Requirements:

Must have a High School Diploma
Good to have a Bachelor’s Degree
