Position Summary
The Home Health Registered Nurse (RN) provides skilled nursing care to patients in their homes, focusing on assessment, treatment, education, and coordination of care. The RN works independently while collaborating with physicians, therapists, and the interdisciplinary care team to promote patient safety, recovery, and independence.
Key Responsibilities
Patient Assessment & Care
Perform comprehensive in-home assessments (physical, emotional, environmental)
Develop, implement, and update individualized plans of care
Monitor patient condition and identify changes requiring intervention
Provide skilled nursing services according to physician orders
Bilingual Spanish/English preferred
