Responsibilities

- Conduct medication therapy management to optimize patient outcomes.
- Monitor patient progress and adjust medication regimens as necessary.
- Maintain compliance with HIPAA regulations to protect patient confidentiality.
- Manage inventory effectively, ensuring the availability of medications while minimizing waste.
- Provide education on physiology knowledge relevant to medication therapy.
Requirements
- Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
- Valid pharmacist license in the state of Oregon.
- Strong understanding of clinical pharmacy principles and medical management practices.
- Experience in compounding medications is a plus.
- Excellent communication skills with a focus on patient service.
- Ability to work collaboratively within a multidisciplinary team environment.
- Strong organizational skills with attention to detail in inventory management.
