Job Title: Clinical Pharmacist

Position Summary

The Clinical Pharmacist is responsible for providing comprehensive pharmaceutical care to ensure the safe and effective use of medications. This role works closely with healthcare providers to optimize medication therapy, improve patient outcomes, and support high-quality patient-centered care through clinical consultation, medication management, and pharmacy operations.

Responsibilities

  • Assess patient medication needs and provide comprehensive pharmaceutical care and medication management
  • Counsel patients on prescription and over-the-counter medications, including usage, side effects, and drug interactions
  • Collaborate with physicians, nurses, and healthcare teams to optimize medication therapy and treatment plans
  • Conduct Medication Therapy Management (MTM) reviews and recommend appropriate medication adjustments
  • Monitor medication adherence and develop strategies to improve patient compliance
  • Maintain accurate patient records and ensure compliance with pharmacy laws, regulations, and standards
  • Oversee pharmacy operations including medication dispensing, inventory management, and workflow efficiency
  • Support quality assurance and medication safety initiatives within the pharmacy
  • Participate in community health education, wellness programs, and health promotion activities
  • Stay current with advancements in pharmacotherapy, clinical guidelines, and pharmacy practice standards

Qualifications

  • Doctor of Pharmacy (Pharm.D.) degree from an accredited institution required
  • Active Pharmacist license in the applicable state required
  • Strong knowledge of pharmacology, medication therapy management, and therapeutic guidelines
  • Previous retail, clinical, or hospital pharmacy experience preferred
  • Excellent communication, patient counseling, and collaboration skills
  • Proficiency with pharmacy management systems and healthcare technology preferred
  • Strong analytical, organizational, and problem-solving abilities
  • Ability to work efficiently in a fast-paced healthcare environment with strong attention to detail

Education Requirements:

Must have a PhD/Doctor’s Degree
Must have a Bachelor’s Degree
Must have a High School Diploma

Skill Requirements:

Must have 2+ years of experience in Pharmaceutical knowledge
Nice to have knowledge in Inventory management
Nice to have knowledge in Counseling techniques
Nice to have knowledge in Regulatory compliance
Must have 2+ years of experience in Drug interactions
Nice to have knowledge in Medication management
Nice to have knowledge in Clinical guidelines
