Position Summary
The Registered Nurse (RN) is responsible for providing comprehensive patient-centered care through clinical assessments, care coordination, patient education, and ongoing monitoring of patient conditions. This role works collaboratively with interdisciplinary healthcare teams and caregivers to ensure high-quality care and positive patient outcomes in the home health setting.
Responsibilities
- Perform comprehensive evaluations of patients’ physical, emotional, and overall health status
- Develop, implement, and update individualized care plans based on patient needs and physician recommendations
- Educate patients and caregivers on chronic disease management, medications, side effects, and self-care techniques
- Coordinate care with physicians, therapists, social workers, and other healthcare professionals
- Monitor patient progress and communicate changes in condition to the appropriate care team members
- Maintain accurate and timely EMR documentation including assessments, interventions, and patient progress notes
- Supervise Structured Family Caregivers, Home Care Aides, and other caregiving staff to ensure quality care delivery
- Ensure compliance with care plans, agency policies, and healthcare regulations
Qualifications
- Active Registered Nurse (RN) license in the applicable state
- Previous home health or case management experience preferred
- Strong clinical assessment and patient education skills
- Knowledge of EMR documentation and care coordination processes
- Excellent communication, organizational, and interpersonal skills
- Ability to work independently and collaboratively within an interdisciplinary team
