Responsibilities

• Provide patient counseling on medication usage, side effects, and interactions.
• Conduct medication therapy management to optimize patient outcomes.
• Monitor patient progress and adjust medication regimens as necessary.
• Maintain compliance with HIPAA regulations to protect patient confidentiality.
• Manage inventory effectively, ensuring the availability of medications while minimizing waste.
• Provide education on physiology knowledge relevant to medication therapy.

Requirements

• Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
• Valid pharmacist license in the state of Oregon.
• Strong understanding of clinical pharmacy principles and medical management practices.
• Experience in compounding medications is a plus.
• Excellent communication skills with a focus on patient service.
• Ability to work collaboratively within a multidisciplinary team environment.
• Strong organizational skills with attention to detail in inventory management.
